Patient questions
Ask us a question
Pick a question below and you will get the same answer our front office would give you. For anything about your own health, care, or medications, call us at (405) 756-1414.
Cost, insurance, and billing
What insurance do you accept?
We generally accept Medicare, SoonerCare, and most private insurance plans. Please bring your insurance card to every visit.
One thing to know: even when we accept your plan, not every provider in our clinic is covered by every plan. Coverage can be different from one provider to the next. Call us with your plan name before your visit and we will check for you.
No insurance? You can still be seen. Reach out to our Community Health Worker at (405) 756-1414 ext. 408. They will help you find out — at no cost — if you qualify for SoonerCare or a plan through the Health Insurance Marketplace.
I don’t have insurance. What will my visit cost?
You have two ways to pay less.
Sliding fee program. If your household income is at or below a set limit, we lower your cost based on your income and family size. Bring proof of income to apply. If your income is low enough, you’ll pay only a small set fee.
Same-day discount. If you’d rather not apply, or your income is above the limit, you can pay a lower rate by paying in full on the day of your visit. Ask the front desk what your visit will cost.
You can also ask for a written estimate before a scheduled visit.
Either way, no one is turned away for being unable to pay.
What should I bring to my first visit?
- Your photo ID
- Your insurance card, if you have one
- A list of the medicines you take
- A way to pay your copay or visit fee
- Proof of income, if you want to apply for the sliding fee program
Proof of income can be recent pay stubs, last year’s tax return, a W-2, a letter from your employer, or an award letter for benefits like SNAP. Didn’t bring it? That’s okay. Bring it later and we can go back up to 30 days to lower those bills.
Who do I talk to about my bill?
Call us at (405) 756-1414 option 6 and ask for billing. We can go over your charges, set up an interest-free payment plan, or check whether the sliding fee program can lower what you owe.
If you don’t have insurance, you can also ask for a written estimate before a scheduled visit. If your bill ends up $400 or more above that estimate, you may be able to dispute it. Ask us and we’ll walk you through it.
Visits and appointments
How do I schedule an appointment?
You can schedule an appointment by calling (405) 756‑1414. When the phone menu starts, just choose the option for the clinic you want:
- Option 2: Lindsay (Main Street)
- Option 3: Leopard Nation Wellness Station
- Option 4: Chickasha
- Option 5: Pauls Valley
Our team will help you get scheduled quickly.
What happens if I am late or miss an appointment?
Please call us as early as possible. We understand things come up, and a quick call helps us keep the day running smoothly for everyone.
Arriving more than five minutes past your appointment time may mean we need to reschedule your visit if the provider is unable to see you. Coming about fifteen minutes early is the best way to make sure you’re seen on time and gives us a moment to update any important information.
Missing an appointment without calling ahead counts as a no‑show. If a patient has three no‑shows within a six‑month period, they will be placed on a walk‑in‑only basis for 90 days.
What should I do if I need medical help after hours?
SCMRC offers after‑hours clinical support so you can still reach a medical professional when our clinics are closed. Simply call our main line at (405) 756‑1414, and you’ll be connected with our after‑hours team for help.
If it’s a life‑threatening emergency
Please call 911 or go to the nearest emergency department right away.
