This website policy is different from our HIPAA Notice of Privacy Practices. The Notice of Privacy Practices explains how SCMRC may use and disclose protected health information and how patients can exercise HIPAA rights. The full notice is available at https://okscmrc.org/notice-of-privacy-practices.

Protected Health Information
Information SCMRC creates or receives in providing health care may be protected by HIPAA and other health privacy laws. Our use and disclosure of that information are governed by our Notice of Privacy Practices, not solely by this website policy. Third Party Services and Links
Our website may link to third-party websites, maps, social media, payment services, patient portals, or other services. Their privacy and security practices are governed by their own terms. A link does not mean SCMRC controls or endorses those practices.
Data Security and Retention
We use reasonable administrative, physical, and technical safeguards appropriate to the nature of the information. No website, email, text message, or storage system can be guaranteed completely secure. We retain information for as long as reasonably necessary for the purposes described, to meet legal and operational requirements, and consistent with applicable retention schedules.

A parent or legally authorized person should contact SCMRC if they believe a child submitted information through the public website without appropriate permission. Access to a minor’s medical information is governed by applicable law and the Notice of Privacy Practices.
